Burn Mark on Humu Trigger?

I brought home a beautiful baby humu picasso triggerfish 2 weeks ago and still have him in my QT before adding him to my display tank. He looks awesome and he’s a boisterous eater now, but I noticed he had a strange burn looking mark on his side and one of hie eyes was slightly cloudy when I brought him home. It’s somewhat healed and his eye’s no longer cloudy, but what was the cause of this? Perhaps ammonia burn from the LFS or maybe bad collection practices? Maybe a disease, but I highly doubt it since if it was a disease, he’d probably be dead by now and I would’ve known if his condition was getting worse. any ideas to what it could be?
